Hospital to Home
Coming home after a hospital stay can be overwhelming. RQ Living is here to make the transition smooth, safe, and supported.
Our Hospital to Home service is designed to help you regain independence and confidence as you return to familiar surroundings. We work closely with hospitals, your healthcare team, and the NDIS to coordinate everything you need—from setting up supports at home to making sure your plan reflects your current needs.
